Working aloft unprotected. Two crew members were washing down the same hull, over the side, from the sundeck (CHIRP report M2762). One wore a harness, clipped to a fixed point. The other did not, and had no shoes on either.
The equipment existed. The procedure was known. One person simply was not using it, and nobody on deck said anything.
CHIRP's comment is blunt about where the fault sits: a procedure not consistently followed is a system weakness, not a workforce failure. The gap was not missing kit, it was missing supervision willing to say something in the moment.
