Passage plans and. A safety audit on a large private superyacht (CHIRP report M2846) found procedures that read well and matched almost nothing on board: manuals referring to an ECDIS that was not fitted, charts a year out of date, life rafts without valid hydrostatic release units, and no fall-arrest system for crew who went aloft on improvised ladders.

Some of the procedures had been generated with AI tools and never checked against the actual vessel. No emergency drill had been run before the audit.

CHIRP's line on it: if safety can be demonstrated on paper but not on deck, it is not safety, it is paperwork. The captain's own decision to invite an outside audit, and to run drills during the visit, is the part worth copying.
