Near miss and reporting concerns

Near miss and. A reporting facility existed. The deck crew still had no practical or confidential way to use it.

The only route was a shared laptop in an area normally occupied by senior personnel. Two crew members discussed the near miss privately, but did not feel comfortable raising their concerns openly. Alternative reporting arrangements had been requested and were not implemented.

On board, confidentiality can fail at the point of access. The form may be available, but the route to it can make the reporter visible before anything is submitted.

A confidential reporting system is only a barrier if the crew can use it in practice.

Where would a ship ratings physically go today to submit a safety concern without making the report visible to others?
